Practice Policies

  • Registration policy 

    • All patient forms and consents must be completed 24 hours prior to initial evaluation

  • Medication Policies 

    • If in need of a refill, contact Condon Psychiatry through the patient portal at least 2 business days in advance. Please do NOT assume we received a pharmacy request, as these automated faxes/messages are often lost.  

    • Refill requests via voicemail or telephone will not be fulfilled. 

    • Refill requests will not be honored after missed appointment until patient is seen again 

    • Condon Psychiatry values a safe and evidence based approach to medication management.  As such we do not routinely start patients on benzodiazepines or stimulants.

      • Condon Psychiatry adheres to strict prescribing policies for benzodiazepines and stimulants which include the following: 

        • Toxicology screens upon initiation and at any time after initiation.

        • No early fills.

        • No concurrent prescribing of benzodiazepines and stimulants. 

        • No concurrent prescribing of controlled substances with cannabis use.

        • If you are found to be receiving controlled substances from multiple providers you will be discharged

    • Condon Psychiatry must be your sole prescriber for all psychiatric medications.  If you are receiving psychotropics from multiple providers you will be discharged.

  • Appointment Policies 

    • 24 hour advance notice is required if you need to reschedule.  Otherwise, there will be a $150 late cancellation fee.  

    • Not attending an appointment, including if you contact Condon Psychiatry AFTER the start time of an appointment, you will owe the full out of pocket fee for the service ($150 for a follow-up, $300 for an evaluation).

      • Appeal Process: If a patient believes there are extenuating circumstances regarding fees, they may submit a written appeal to the practice. The appeal will be reviewed, and a response will be provided within a reasonable timeframe.

      • Exceptions: Exceptions to this policy may be made for emergencies or unforeseen circumstances. Our practice will use discretion and assess situations on a case-by-case basis.

    • Condon Psychiatry allows a 10 minute grace period to arrive for appointments.

    • 2 missed appointments or 2 late cancellations will result in discharge from the practice.

    • All intake forms must be completed at least 24 hours prior to initial appointment.

  • Financial Policies 

    • Existing patients must keep a credit or debit card on file in our secure system.

    • New patients must put a credit or debit card on file in our secure system for appointment fees.

    • Fee for service patient’s cards will be charged the fee at time of appointment and superbill will be provided to the patient through email or patient portal for insurance reimbursement.

    • Fee for service rates are $300 for evaluation and $150 for follow up

    • Insurance fees will be processed through Headway.

    • All patients with insurance must register with Headway for appointment coverage or they will be charged at fee for service rates.

    • If you have an unpaid balance greater than 30 days old your follow up may be canceled until the balance is paid.

    • Unpaid balances may be sent to collections after Condon Psychiatry attempts to collect for 90 days.

  • Communication

    • All patient communication and telehealth visits will take place through IntakeQ’s web portal.  This is a secure and HIPPA compliant platform.  IntakeQ will keep a record of all communications for patient and provider reference.

  • Safety policies 

    • Threatening speech or behavior will not be tolerated and will lead to immediate discharge from the practice.

    • Emergency and Crisis Intervention Policy

      • In the event a patient presents as a threat to self, others, or is gravely disabled and we are unable to make an adequate plan for safety:

        • Emergency contact will be contacted.

        • Crisis intervention or emergency services will be contacted to ensure patient safety.

      • Condon Psychiatry does not provide after hours or weekend coverage.  By signing this form you agree that if you are having a psychiatric or medical emergency you will not contact Condon Psychiatry but rather seek emergent care by calling 911 or 211 for mobile crisis, or going to the nearest emergency department.  

      • If you require a higher level of care Condon Psychiatry will coordinate with those care providers and receive clearance from them prior to returning to outpatient level of care.

  • Misc 

    • Office Hours are 8:00 AM to 4:00 PM Monday through Thursday

      • There is no after hours or weekend coverage so if you contact Condon Psychiatry during off hours there will be no response until office hours start again

    • There is a $50 fee for each instance of FMLA paperwork being completed. 

    • Condon Psychiatry DOES NOT complete social security disability paperwork but you can request your records at anytime if you believe it will help you case.

    • Condon Psychiatry DOES NOT certify service, therapy, or emotional support animals
